    Summary



    The Chief Nurse (CN) for Home and Community Based Care (H&CBC), reports directly to the Deputy Associate Director, Patient Care Service (DADPCS)/Deputy Executive Nurse and will be an important member of the Patient Care Services Executive Management Team.

    Duties


    The CN will be a program manager for the nursing assets that support the following departments: Community Nursing Home, Community Adult Day Health Care, Home Telehealth, Medical Foster Home, Community Care Network Rehabilitation, Care Coordination Office, Hospital in Home, Complex Transition Care, Veteran Directed Care, Home Oxygen, Home Based Primary Care, Respite Care services, Music and Memory, Caregiver Support Program, and Community Residential Care.

    The CN will directly supervise managers to include program managers of these areas with oversight and accountability for all nursing care activities related to quality and resource utilization within Patient Care Services (PCS).

    The CN will provide clinical and administrative leadership and oversight for both clinical nursing practice and administration; establish/promote optimal standards in care delivery and policies/procedures which reflect the current evidence-based practice; work with his/her peers to formulate strategic directions for PCS; plan, implement, and lead activities to enhance customer service; and collaborate with other health care disciplines to establish and maintain programs that cross service and/or discipline lines and influence organizational mission and overall health care delivery.

    As the CN for H&CBC, the incumbent will be responsible for assessing and recommending the allocation of resources, both human and material, necessary to ensure positive outcomes of existing programs and new projects.

    The incumbent will support quality, safety, value, and efficiency; striving to ensure that Veterans receive health care services in the appropriate level of care, when that care is needed, and also receive only needed health care services.

    The CN for H&CBC will use performance improvement data, e.g.

    staffing effectiveness, to design delivery models, ensure budgetary responsibility, and establish staffing levels that will meet the needs of the Veteran patient.

    The CN will be responsible and accountable for the delivery of high quality, customer-focused, cost-effective patient care to meet the mission and goals of current operations at South Texas Health Care System.

    The incumbent will facilitate interdisciplinary teamwork to assure optimal quality in all care delivery processes, promote productivity/efficiency, monitor clinical outcomes, work to enhance patient satisfaction levels and be responsible and accountable for continuous performance improvement within PCS.
