- Conduct Adult/Geriatric Assessments, including medical history reviews, diagnosis, treatment, health education, physician referrals, and case management according to Matrix guidelines
- Collaborate with healthcare professionals, family members, and caregivers to ensure comprehensive care
- Work closely with Primary Care Physicians to educate patients and provide follow-up care
- Deliver services in various settings such as homes, skilled facilities, and through telehealth visits
- Master's Degree or equivalent experience with NP licensure
- Master's Degree or equivalent experience with PA licensure
- Valid RN and NP or PA licensure with prescribing authority
- Board certification in a Matrix-approved specialty
- Current BLS, ACLS, or CPR certification
- Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant and willingness to travel
- Proficiency in EMR software and strong communication skills
- Positive, team-oriented attitude with adaptability to change
- Valid driver's license, reliable transportation, and willingness to travel
- Travel is essential to reach patients in their living environments

About the Position:
During a comprehensive health assessment that can last up to an hour, Matrix providers evaluate a member's current health status, medical history, medication adherence, social environment, and other factors that may pose risks. This in-depth assessment provides valuable insights into a member's overall well-being, which are often overlooked in standard office visits. The Matrix Comprehensive Health Assessment aims to enhance the quality of care and address multiple care needs in a single visit.

Nurse Practitioner or Physician Assistant - PRN/Part Time - Home Risk Assessments
Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $90.00 or $115.00 per completed Assessment based on county
Location: Home, SNF, Telehealth, and other settings in Bronx, New York
Hours: Flexible scheduling, including days, evenings, nights, and weekends
Benefits: PRN/Part Time positions offer select benefits like 401(k) and voluntary benefits, including mileage reimbursement
