    • Friday from 9:00AM 5:00PM*MUST BE COVID 19 VACCINATED OR HAVE MEDICAL/RELIGIOUS EXEMPTION*Position SummaryUnder the direct supervision of the Community Administrator or Executive Director, this position is responsible for developing and linking social service provider organizations with residents of a geriatric retirement community and their families, consulting with families, residents and community management as needed.
    Develops an effective case management and advocacy plan for assigned residents and monitors its implementation.

    Establishes and maintains effective working relationships with internal and external resources and agencies involved with the resident's intake, assessment, data collection and treatment.

    Assists in developing data collection forms and ensures accurate and complete documentation of required information. Participates in or conducts related training activities. Performs related responsibilities as required.
    Key ResponsibilitiesProvides general case management, intake and referral services to all residents of a community.
    Establishes linkages with agencies and service providers in the community and negotiates best service pricing.
    Sets up a directory of providers for use by both staff and residents. May set up volunteer support programs with service organizations in the community.
    Develops case plans in coordination with community assessment services.

    Refers and links residents to service providers in the general community for case management, personal assistance, homemakers, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness and legal advocacy.

    Educates management team on issues related to aging in place and service coordination to help them to better work with residents.

    Trains community residents in the obligations of being a resident or coordinates such training. Educates the residents on service availability, application procedures, client rights.
    Assists residents build informal support networks with other residents, family and friends.
    Demonstrates professional work behavior, following Wesley Woods policies and procedures.
    Maintains current knowledge federal and state regulations. Attends continuing education classes as required.
    QualificationsMinimum QualificationsCollege degree in Social Work, Gerontology, Psychology or Counseling.
    Two to Three years' experience in social service delivery required, preferable with senior citizens. Experience navigating state agency initiatives and programs.
    Supervisory or management experience preferred.
    Demonstrated knowledge of the aging process, elder services, disability services, and support services for the elderly.
    The ability to read, write and follow written and oral instructions in English.
