The Avenue at Strongsville is a Post-Hospital Rehabilitation and Skilled Nursing Care Facility that offers a distinctive environment not commonly found in skilled nursing facilities. Delegation of Authority: The person in this position has the authority to:

. Complete MDS asessments in complance wit federal and stae guidelines

. Review and certify completion of portions of the MDS assessments completed

. Complete the RAPS for the MDS, if qualified.

. Participate interdisciplinary meetings

Duties: Performs all duties within the guidlines of Progressive Quality Care

. Consistently demonstrates responsibility, accountability, and dependability

. Establish lines of communication with other departments and maintain a cooperative atmosphere

. Schedule resident MDS assessments to ensure timliness

. Assume responsibility for utilization of the Prospective Payment System guidelines in completeig MDS assessments

. Interview residents, residents' representatives and others for information related to the minimum data set assessment

. Review medical records and records related to residents' health status to collect information related to the MDS

. Observe resident's to collect information related to the minimum data set assessment

. Enter data into the computer program for minimum data set assessments

. Electronically transmit Minimum Data Set assessments in accordance with federal and state guidelines

. Print MDS and related reports from the computer program

. Review state and facility generated reports for the MDS assessments and take follow-up action as necessary

. Observe facility safety policies and procedures

. Assure that the Resident's Bill of Rights is followed when asssessments are done

. Participate in tracking changes in the resident's condition to assure the timely completion of assessments

. Review resident's care plans to be sure that all information is included

. Participate in the Quality Improvement/Quality Assurance Program, as requested

. Assist the Director of Nursing in the review of documents to assure compliance with federal and state guidelines

. Work cooperatively with the Director of Nursing to maintain compliance with the nursing service budget

. Notify the MDS Assessment Coordinator os any resident needs and/or concerns that are outside the employee scope of practice

. Participate in committee meetings and function on committees as assigned

. Complete mangement reports and special assignments as assigned

. Assist in education staff in appropriate documentation to assure compliane with federal and state guidelines

. Participate in interdisciplinary care conference meetings and other meetings related to care of the resident, as requested
