    Job Summary

    Reporting to the Clinical Manager or designee, the Registered Nurse (RN) will work with diverse populations with multifaceted needs. The RN will serve as a care planner, care coordinator, advocate and record keeper for the patients of THC and their families. They are responsible for providing nursing care through, nursing assessment, teaching, nursing plans of care and routine nursing procedures. The nurse-patient/family relationships, continuity of care and accountability are central to the delivery of individualized high quality patient care. This position works to plan and coordinate the delivery of excellent patient care in collaboration with the physician, other healthcare providers, department support staff, and other disciplines.

    Contacts and interactions vary and may involve multiple constituencies such as direct interaction with THC's executive management, community organizers, the general public, THC's patients, physicians, colleagues, assigned staff, vendors, contractors and consultants for the purpose of providing and exchanging information.

    Example of Essential Job Functions

    • Provides care in collaboration with the patient, THC healthcare team, outside providers, and family as appropriate.
    • Acts as a patient advocate. Serves as a liaison between patients, their families and healthcare providers and health care team to improve patients' quality of life through collaboration and follow up care.
    • Demonstrates the knowledge and skills necessary to provide expected standards of care based on physical, psychosocial, cultural, educational, spiritual, safety and age-appropriate considerations of patients.
    • Develops, implements and evaluates individualized patient care plans and facilitates coordination of care across the continuum.
    • Demonstrates strong organizational skills that maintain and coordinate safe delivery of quality care.
    • Maintains oversight over the clinic to ensure safe patient care by maintaining logs, checking expiration dates of medications and medical surgical supplies, maintaining emergency cart and ensuring compliance with the Maryland VFC Program.
    • Takes full and ultimate responsibility for the quality of nursing care he/she provides to individuals and groups, including but not limited to health maintenance, teaching, counseling, collaborative planning and restoration of optimal functioning and comfort.
    • Performs medication administration, venipuncture, clinical nursing assessment/triage, and other clinical nursing skills as required.
    • Provides delegation and supervision in accordance with the Nurse Practice Act – Provides leadership and oversight of the health care team to ensure efficient, quality care is provided by the members of the healthcare team.
    • Assures timely and efficient flow of the clinic. Prioritizes tasks efficiently and effectively and completes Patient Cases and Follow Up in a timely fashion according to THC policy.
    • Ensure medical care and follow up are planned and documented and that all requests for services are completed for each patient upon exit.
    • Documents appropriately in the EMR. ie nurse visits and triage/walk ins if necessary.
    • Assists with timely completion of forms and documents needed by patients, families or other required documents.
    • Communicates regularly and as needed with Clinical Manager. Keeps manager aware of any concerns in the clinic.
    • Provides quality care and customer service to the patients and their families, regardless of their socio-economic status.
    • Provides excellent customer service to internal and external customers, patients and their families under all circumstances, exercising conflict resolution skills with customers and staff when necessary.
    • Demonstrate autonomy by utilizing the nursing process to assess patients and determine next steps in the delivery of care.
    • Practices within the Nurse Practice Act, ANA standards of Practice and Code of Ethics
    • Demonstrates THC core values that reflect the mission of THC.
    • Practices in accordance to Total Health Care policies, protocols, procedures, and workflows.
    • Maintains a current level of nursing knowledge and skills by completing all mandatory training and scheduled competencies.
    • Maintains compliance with infection control procedures and regulatory requirements
    • Maintains and uphold all regulatory requirements including HRSA, UDS, and PCMH.
    • Maintains HIPAA regulations.
    • Attends staff and department meetings as needed.
    • Performs other duties as assigned.
    Minimum Education, Training and Experience Required

    Current Maryland RN licensure, graduate of an accredited nursing program and one year minimum experience in an acute care facility required. BSN preferred. Current BLS certification required.

    Knowledge of FQHC operations, operating principles, guidelines and bylaws. Excellent leadership, customer service, organizational and presentation skills as well as the ability to effectively communicate THC's vision, and motivate others to achieve it organizationally, departmentally, and personally/professionally. Ability to communicate effectively (verbally and in writing). Ability to plan and organize work initiatives to successfully accomplish center/organizational goals and objectives. Ability to multi-task, prioritize and delegate as appropriate. Strong analytical, problem solving and interpersonal skills. Ability to identify, develop and implement short/long-term strategic goals and objectives. Ability to develop and maintain customer relationships; influence, build credibility and trust. Ability to think critically as well as apply critical thinking skills. Ability to: ensure and advocate for quality healthcare and services; and, lead and manage a diverse staff.

    Licenses and Certifications

    Nursing License issued in the state of Maryland

    Current CPR certification
