Job DescriptionJOB SUMMARY
The Clinical Nurse Liaison works under the supervision of the Director of Business Development with clinical supervision from Patient Access Manager. Develops well-informed, responsive relationships through business development efforts and marketing with referral sources. Facilitates referrals by gathering and interpreting medical and financial data, meeting with patients/families/case managers/providers to ensure optimal transition to LCFL services, coordinating transfers and discharges, assessing appropriateness, and beginning admission if appropriate. Acts as a resource/consultant to assess patient needs, through effective communication and education regarding the various programs offered by LCFL. Works to develop and maintain business partnerships, increase census and length of stay. Generates referral growth through effective sales contacts, calls, and presentations. Participates in various aspects of outreach, marketing, education, networking, and community relations. Responsible for meeting set goals. Responsible for overall development and referral growth of assigned territory. Supports the development and execution of agency marketing and engagement plan. The Professional Registered Nurse is also responsible for continuing his/her own personal development to enhance practice and the profession of Nursing. Works with Palliative Care Nurse Practitioners and Physicians. Performs other duties as assigned.
PRIMARY JOB DUTIES
1. Acts as a marketing, sales liaison, and referral source to physicians, facility staff, hospital staff, patients, and families when appropriate, and other health care providers regarding end-of-life issues and palliative care.
2. Acts as a resource for referral partners to assist with assessing patients for appropriateness of admission to LCFL programs.
3. Coordinates transition of hospital inpatients, facility patients, and home care patients to the appropriate LCFL program.
4. Analyzes trends and patterns in referrals and admissions to identify needs and to develop marketing strategy.
5. Actively identifies barriers to service in the community, physician offices, facilities, and hospitals; takes an active role in solutions/resolutions by providing excellent customer service.
6. Acts a liaison between LCFL and referral sources by providing service information to physicians within hospital, long term care and assisted living settings as well as patients and families and other providers of health care.
7. Coordinates engagement strategies and activities for consistent community outreach, education, awareness, and involvement.
8. Organizes and performs work effectively and efficiently.
9. Maintains and adjusts schedule to enhance program performance.
10. Demonstrates positive interpersonal relations in dealing with all members of the team and works in conjunction with the Physician/Nurse Practitioner.
11. Maintains and promotes customer satisfaction
12. Effectively demonstrates the mission, vision, and values of Lower Cape Fear LifeCare on a daily basis.
JOB SPECIFICATIONS
1. Education: Graduate of an accredited school of nursing, either through an AD, Diploma or BSN program. Degree in business, marketing, or related field preferred.
2. Licensure / Certifications: Current license to practice professional nursing in the State of North Carolina or South Carolina depending on position location.
3. Experience: Two years experience. Prefer hospice or home care. Prefer experience in marketing, public affairs, community relations or hospital liaison nurse.
4. Essential Technical / Motor Skills: Ability to speak clearly to communicate with patients, families, physicians, and staff. Good command of the English language. Computer experience helpful.
5. 5. Interpersonal Skills: Ability to develop positive interaction with patients, families, physicians and staff and other health professionals to effectively communicate and educate about hospice services and end-of-life care. Strong verbal and written communication skills. Highly self-motivated and self-disciplined. Flexible.
6. Essential Physical Requirements: Ability to move freely. Ability to work long hours and manage stressful situations.
7. Essential Mental Abilities: Ability to define problems, collect data, establish facts, and draw valid conclusions. Requires higher level of mental faculties accompanied by short and long-term memory.
8. Essential Sensory Requirements: Touch, sight, hearing, ability to organize thoughts, facts, and ideas. Ability to translate the written word.
9. Exposure to Hazards: May be exposure to blood and body fluids, infectious diseases, needle puncture wounds. May encounter patients and other situations which present a potential threat to personal safety.
10. Hours of Work: Flexibility with schedule required. Weekends, holidays, evening, and early morning hours will be required from time to time.
11. Population Served: Physicians, RN's, facility administrators, discharge planners, social workers, other health professionals, insurance specialists, public, community leaders, vendors. All patient populations from infants through geriatric with life limiting illness.
12. Driving: Will be required to drive. Must have reliable transportation. Will be subject to the Safe Driver Policy.
