    We are hiring a full-time Physical Therapist to join our team at Nason Hospital Home Health, part of LHC Group.

    Responsibilities

    The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.

    • Evaluate patient's functional status and physical therapy needs
    • Assist physician in developing therapy plan of care
    • Observe and report patient's response to treatment and any changes in condition
    • Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice
    • Instruct and inform healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals

    Requirements

    To be successful in this role, you must have:

    • Current Physical Therapy licensure in state of practice
    • Current CPR certification
    • A current driver's license and vehicle insurance, and access to a dependable vehicle
