- CPR Certified
- Certification in Hospice & Palliative Medicine (CHLPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime or inclement weather.
- Valid Driver's License with Safe Driving Record

What You Should Know About the LPN Team Float Role:The LPN Team Float provides quality nursing care to patients in a manner that meets the patient and family needs working under the direction and supervision of an RN Care Manager.
Qualifications:
LPN's - Registered LPN with the State of Ohio without any board actions. One year of previous nurse experience.
