    The Mental Health Therapist , Rapid Response Team provides rapid response to enrolled LifeWorks NW (LWNW) clients who are hospitalized in acute psychiatric units or who are seeking services through a hospital Emergency Department (ED). Provide transitional services for enrolled individuals transitioning back to their mental health or addictions program. Provide crisis intervention services to individuals in EDs to divert them from hospitalization.

    Provide behavioral health assessments for clients seeking services post hospital. Run intake clinics for clients who have missed previous assessment appointments, run drop-in clinics for clients who need support between sessions, and run engagement groups for clients who have become disengaged from their outpatient clinical program.

    Clinicians primarily work with adults and meet with clients in hospitals, emergency departments, in the community and clinic settings.

    Location: This position is based out of LifeWorks NW's Beaverton site.

    Pay/Benefits: $62,229 starting annual salary plus, Full benefits .

    Essential Responsibilities:

    • Clinician works primarily with adults, providing transitional services to clients who are recently hospitalized or who are not firmly engaged with their program. Clinicians in this position work independently across several program sites, providing expedited intake assessments and assignment to LifeWorks clinicians for ongoing care, as well as providing other support services necessary to ensure client engagement in the adult outpatient programs.
    • Provide behavioral health assessments in hospitals or at a clinic site for clients who are new to LifeWorks NW. Assign clients to an on-going clinician and provide support until the first appointment with that clinician.
    • Assure that hospitalized individuals are seen within seven days of hospitalization. Follow up with individuals who don't show for their first scheduled appointment following hospitalization.
    • Run 'drop-in clinics', meeting with clients who need additional support between visits to their ongoing clinician.
    • Run engagement groups, utilizing a manualized approach to assist disengaged clients in reconnecting better to services.
    • Manage a caseload of adult outpatient clients who are receiving "aftercare" services, conducting periodic wellness checks with these clients to monitor stability, provide support, and if necessary, provide short-term therapy (individual or group).
    • Manage a caseload of medication-only adult outpatient clients who have been shifted from therapeutic services to medical services. Conduct periodic wellness checks with these clients to monitor stability and provide support, and if necessary, short-term therapy (individual or group).
    • Clinicians meet with clients in hospitals, emergency departments, the community and clinic settings coordinating services with hospital and ED social workers as well as other clinicians throughout LWNW's range of clinical programs. Performs client evaluations and risk assessments. Develops a short-term stabilization or safety plan to provide a stable environment for clients. Completes written narrative of evaluation and obtains necessary releases. Coordinates with clients' treatment team. Maintains awareness of LWNW programming including protocols and community referral resources. Helps to build positive relationships with other community providers and provides appropriate referral services.
    • Provides mandatory abuse reporting as necessary.
    • Arranges transportation for clients to attend clinical and community appointments.
    • Maintains professional presentation in the community at all times and responds to all situations / calls with a customer service attitude.
    • Prepares mental health evaluations and reports for all client contacts. Maintains clear and concise records. Maintains case files that include activities, progress notes, evaluations, and recommendations according to confidentiality guidelines. Completes documentation in an electronic health records system. Completes time and attendance forms.
    • Travels between sites and in the community, may transports clients in personal vehicle.
    • Requires some evening and weekend shifts.
    • Qualified licensed clinicians may be requested to provide licensure supervision as part of their regular position in compliance with the rules of their licensing board.

    Qualifications:

    • This position requires a valid and active QMHP credential certification through MHACBO or a valid and active status (licensed or registered intern) with an approved Oregon licensing board.
    • Master's degree in social work, Psychology, Counseling or a Mental Health related field.
    • Demonstrated ability to conduct a mental health crisis assessment, including identifying precipitating events, gathering histories of mental and physical health, alcohol and other drug use, past mental health services and criminal justice contacts, assessing family, social and working relationships and conducting a mental status examination; complete a five-axis DSM diagnosis; write and supervise the implementation of an Individual Service and Support Plan; and provide individual, family and/or group therapy to address chemical dependency and/or mental health problems.
    • More than 1 year of experience preferred in a broad range of clinical interventions and mental health assessments with children and families or individual with severe and persistent mental illness.
    • Demonstrated ability to work effectively on a highly interdependent team, necessary skill sets include initiative, dependability, flexibility and strong organizational skills.
    • Knowledge of community resources and treatment agencies enabling appropriate referrals for specific client needs.
    • Knowledge of medications commonly used by client population.
    • Requires valid driver's license and verification of safe driving record from DMV.
