    Job Title: Case Manager

    About the Role

    The Case Manager plays a crucial role in assisting seniors with accessing necessary services and supports to live independently in Senior Living Apartments. This position requires strong organizational skills, sound judgment, and excellent communication abilities.

    Key Responsibilities:

    • Provides linkage and referral to services for tenants and families.
    • Completes documentation of all tenant-related services provided.
    • Conducts monthly site visits to senior sites to provide ongoing case management services.
    • Participates in case conferences and training programs/services with public and private agencies.
    • Obtains at least 15 hours of approved in-service training annually.
    • Assists tenants and participates in tenant move-outs as needed.
    • Develops resident service plans if needed and reviews periodically.
    • Maintains an up-to-date knowledge of senior tenant issues, including Medicaid, Medicare, HUD changes/revisions regarding Case Management, and other community programs.
    • Supervises students, volunteers, aides, or other individuals as assigned.
    • Maintains confidentiality and adheres to agency and departmental policies when handling sensitive information.
    • Represents benefit eligibility interviews – e.g., Medicaid, Food Stamps, Medicare, Social Security.
    • Represents community organizations on behalf of the agency.
    • Provides information and referral to community members as needed.
    • Advocates on behalf of tenants and families.
    • Makes decisions concerning services and supports for seniors considering independent living.
    • Educates tenants, families, and staff on available Community Resources.
    • Serves as a liaison between community agencies, tenants, and families.
    • Monitors service delivery, including routine telephone consultations with tenants.
    • Provides crisis management services as needed.
    • Compiles and maintains Resource Center information.

    Requirements:

    • Bachelor's degree in social work, psychology, or human services from an accredited institution.
    • Two years of social work/experience or equivalent service coordination experience with seniors.
    • Working knowledge of the aging process and awareness of community services and programs.
    • Ability to organize, plan, and manage caseloads.
    • Sound judgment, flexibility, dependability, and good writing skills.
    • NYS Driver's License that meets agency policy.
    • Ability to handle multiple tasks simultaneously.

    Supervisory Responsibilities:

    • Oversight of students, volunteers, aides, or other individuals as assigned.
