    JOB PURPOSE:

    PruittHealth Physican Services endorses AMDA's key roles for post-acute/long-term care modified for our setting and model.

    1)Medical Practice Leadership: Responsible for overall facility care and clinicalpractice.

    2)Care Process Leadership: Applies clinical and administrative skills to guide facility careprovision.

    3)Quality of Care: Helps develop and manage quality and safety initiatives, including riskmanagement.

    4)Education, Information, and Communication: Provided to help facility staff, practitioners, and the community to better provide and/or understandcare.

  • KEY RESPONSIBILITIES:

  • Administrative:
  • Participates in administrative leadership, management, and decision-making.
  • Demonstrates leadership to supervise healthcare professionals, and demonstrates exceptional communicationskills.
  • Maintains a working knowledge of applicable federal, state, and local laws and regulations, standards of conduct, as well as applicable PruittHealth policies and procedures in order to ensure adherence to these in a manner that reflects honest, ethical, and professional practice and behavior.
  • 2.Professional Services:

  • Assists in establishing agreements or relationships with outside health care organizations (, regulatory agencies, professional groups, insurers, ambulance companies, andhospitals).
  • Helps ensure adequate documentation of patient care and relatedinformation.
  • Advises the facility on interacting with utilization revieworganizations.
  • Develops policies and procedures related to health care trainees in the facility (, physicians in training, medicalstudents).
  • 3.Quality Assurance and Performance Improvement:

  • Participates in processes which ensure quality of care and which track whether care is effective, efficient, safe, timely, patient-centered, and equitable. This alsoincludes:
  • Appling knowledge of state and national care standards to help meet the applicable care standards.
  • Monitoring PPS provider performance and practice; providing feedback on facility clinical staff performance andpractice.
  • Helping to ensure that QAPI addresses issues that are germane to the quality of patient care and facilityservices.
  • Helping the facility to use QAPI program findings, as appropriate, to update and improve policies, procedures, andpractices.
  • 4.Education:

  • Participates in developing and disseminating key information andeducation.
  • Serves as a resource regarding geriatric medicine and other care-related topics, and helps the staff and practitioners identify and access relevant educational resources (, books, periodicals,articles).
  • Implements professional development programs and clinical education for nursepractitioners.
  • Oversees coordination and execution of Nurse Practitioner clinicalorientation.
  • 5.Employee Health:

  • Participates in surveillance and promotion of employee health, safety, andwelfare.
  • Advises the facility about infectious disease issues related toemployees.
  • 6.Community:

  • Articulates the facility's mission to thecommunity.
  • Acts as an advocate for the facility, encourages and facilitates community involvement in the activitiesofthefacility,andhelpsthefacilityeducatethecommunityaboutitscapabilitiesand services.
  • 7.Rights of Individuals:

  • Participates in establishing policies, procedures, practices, and an atmosphere promoting and assuring the rights of individuals (patients, staff, practitioners, and community) are respected.
  • Ensure that practices reflect and respect resident rights, including the opportunity for self- determination; , via tools such as living wills and durable powers ofattorney.
  • Advocates for the ethical and legal rights of all residents are respected. This includes the right of residents to request practitioners to limit, withhold, or withdrawtreatment(s).
  • Helps the facility accommodate patients' choice of an attendingphysician
  • 8.Social, Regulatory, Political, and Economic Factors:

  • Acquires and applies knowledge of social, regulatory, political, and economic factors that relate to patient care and relatedservices.
  • Helpstoidentifyandprovidecarethatisconsistentwithapplicablesocial,regulatory,political, and economic policies andexpectations.
  • Helps to identify, interpret, and comply with relevant State and Federal laws andregulations.
  • 9.Person-Directed Care Advocate:

  • Supports and promotes a care environment prioritizing self- determination, dignity, choice, and preservation of prior routines andpreferences.
  • Qualifications

    MINIMUM EDUCATION REQUIRED:

    1.Education: MD or DO degreerequired

    2.Completion of accredited residencytraining

    MINIMUM EXPERIENCE REQUIRED:

    1.Minimum3yearsclinicalexperienceasaphysicianinageriatric,adult,orfamilymedicinesetting.

    2.Minimum 2 years in a leadershipposition.

    3.Proven ability to perform comprehensive, adult/geriatric physicalexam.

    4.Demonstrated ability to identify, diagnose, and document chronic disease inadults.

    5.Demonstratedabilitytorecognizeconditions,characteristics,andbehaviorsthatplaceseniorsatrisk.

    6.Proven ability to pursue feedback, continuing education, and professionaldevelopment.

    7.Ability to effectively reconcilemedications.

    8.Demonstrated ability to identify and document the needs of clinically complexpatients.

    9.Pertinent Clinical Work Experience which is relevant to population beingserved.

    10.Ability to understand, articulate, and incorporate palliative and end-of-life principles and practices into care plans and discussions or education with staff, patients, and decision-makers. This includes discussing, clarifying, and documenting prognosis, goals, and advancedirectives

    MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:

    1.Current licensure in state ofpractice

    2.Boardcertification

    3.Current CPRcertification

    4.DEA licensure (Class II-Vunrestricted)

    Requirements – Application:

    1.CAQH-level application and attestation, with credentialing via primary sourceverification

    ADDITIONAL QUALIFICATIONS: (Preferred qualifications)

    1.Geriatric BoardCertification

    2.Hospice and Palliative Medicine BoardCertification

    3.Significant long term care or geriatric practiceexperience

    4.Significant Hospice and Palliative Medicine practiceexperience

    5.Significant medical director experience in long term care and/orhospice

    6.AMDA Certified Medical Director(CMD)

    7.Wound care experience and/orcertification

    8.Bilingual (English &Spanish)

    9.Significant work with behavioral healthpopulations

    10.Significant work with low income and disadvantagedpopulations

    11.Significant work with vulnerable adults or seniorpopulations

    12.Knowledge of communityresources

    Travel Requirements: (travel is required)

    1.Must meet company drivingstandards

    2.Must have valid state driver license and be able to drive acar

    3.Must meet company's automobile liability insurance standards and provide proof ofcoverage

    4.Must be able to independently travel to assignedlocations
