    Residential Case Manager Brick Manor Per Diem Job Summary:

    The Providence Center Residential Case Manager - Brick Manor PER DIEM will provide clinical case management services in order to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Working with consumers to achieve the highest level of independence possible.

    Duties and Responsibilities include the following: Provide case management and support to consumers; Treat consumers with respect; Provide support counseling, problem solving, contracts and limit setting; Establish attainable goals with the consumer; Involve consumer, agencies and others in treatment planning.

    Clinical Responsibilities

    • Collaborate with clinical team on engagement insight and needed supports;
    • Provide support counseling with mental health issues, substance use disorders, medical conditions,
    [Make assessments and provide appropriate crisis interventions as needed]
    Teach clients coping skills for symptom management
    [Teach adult daily living skills] including household chores laundry room cleaning etc [Perform ]household duties as needed.
    Maintain a clean healthy safe environment at all times adhere Infection Control protocols
