    Director of Clinical Services


    We are so excited to announce that BoldAge PACE has a new location opening in 2024 We will open a new facility in Charleston, SC.

    The location will be known as Seamark PACE.


    Position Summary:
    Coordinates and supervises clinical care for participants 24/7 in the center and in participants' homes. Ensures high-quality participant-centered care in accordance with PACE policies, regulation, and best-practices.


    Serves as a member of the PACE program leadership team contributing to policy development, census growth, staff development, and exceptional participant outcomes and satisfaction.

    Clinical Services Manager or a written designee RN shall be in the PACE facility during the hours of operation.

    Basic Qualifications:
    Registered Nurse with leadership skills and experience in healthcare delivery and management. Strong desire to work with the elderly in an innovative, highly interactive, interdisciplinary environment.


    Education:
    Bachelor of Science in Nursing Degree required. Master's degree in management or a healthcare related field preferred. Professional Certification preferred.


    Experience:

    • Minimum 3-years' experience working with the frail and elderly in acute, long term care or community based setting.
    • 3 years progressive management experience required.
    • Experience in home care or long-term care preferred.
    • Experience providing nursing care in a managed care environment preferred.
    • Experience with paperless documentation preferred.

    Licensure/Certification:
    State RN License

    BLS Certification

    Valid driver's license and proof of insurance


    Skills:

    • Ability to maintain a "people first" approach to all decision making, strategic planning and program practices.
    • Knowledge of current concepts and state-of-the-art practices of health services for the elderly, particularly in home and community-based services, and the interdisciplinary team model.
    • Knowledge of managed care environment including best practices for chronic disease management, utilization trends, and implications for the PACE program.
    • Effective communication skills with staff, participants, and contracted vendors
    • Ability and commitment to promote services that exceed expectations and be assertive in creating a culture of "doing the right thing" in all situations.
    • Demonstrated commitment to quality care and services to a frail client population with a focus on continuous improvement and a "do it better" approach.
    • Unwavering commitment to "doing the right thing" evidenced in all decision making and program administration.
    • Ability to promote excellent staff and interdisciplinary team relationships and functioning.
    • Focus on collaboration, shared decision-making, and servant leadership.
    • Competent in conflict resolution and coaching supervision.
    • Understanding of and commitment to self-determination, advocacy, and person-centeredness in the provision of program services.
    • Ability to be creative and innovative in developing systems and making decisions.
    • Tolerance for risk and open-ended situations (Be Bold). Ability to prioritize to align services with goals of care and quality outcomes.
    • Ability to understand and direct staff in addressing highly complex participant situations.
    • Flexibility and adaptability to changing situations and on-going challenges
    • Thorough understanding of established standards of care of the frail elderly and ability to apply best practices to program operations.
    • Understanding of the principles of diversity and inclusion and ability to ensure their incorporation into the culture, policies, and practices of the organization.
    • Knowledge of financial management systems and budgeting processes.
    • Familiarity with management information systems and electronic health records
    • Ability to effectively organize, plan and delegate responsibilities.
    • Demonstrated skills in leadership and decision-making.
    • Demonstrated effective oral and written communication skills.
    • Competence in quality improvement, data tracking and project management
