- Conducting and coordinating the development and completion of resident assessments (MDS) by current rules, regulations, and guidelines
- Ensuring accurate and timely completion of the MDS process
- Collaborating with the interdisciplinary team to develop individualized resident care plans
- Monitoring and documenting resident progress and responses to care plans
- Participating in the Quality Assurance and Performance Improvement (QAPI) process
- Communicating with residents, families, and other healthcare providers to coordinate optimal care delivery
- Staying updated on MDS regulations and guidelines to ensure compliance
