    Job Title: Patient Resource Consultant

    Compensation (Based on experience and qualifications)

    Schedule: Monday - Friday 9:00am - 5:00pm with a Saturday Rotation 8:00am - 1:00pm

    We are seeking a Patient Resource Consultant (PRC) to join our team. As a PRC, you will be responsible for interviewing, verifying insurance benefits, and performing financial case management for uninsured and underinsured patients. You will review, evaluate, and coordinate health insurance benefits available through public and private health care coverage programs for reimbursement. The ideal candidate will be passionate about helping patients understand insurances, payment options, and all the services/programs offered by our healthcare facility. You will work with internal and external stakeholders to provide enabling services to patients, including transportation and translation services.

    Responsibilities:
    • Conduct financial assessment interviews and screen patients for healthcare services
    • Verify insurance benefits and coordinate health insurance benefits available through public and private health care coverage programs for reimbursement
    • Assist and perform financial case management for uninsured and underinsured patients
    • Work with patients to ensure their understanding of insurances, payment options, and educating them on all CFHC's services/programs offered
    • Work with internal and external stakeholders to provide enabling services to patients, including transportation and translation services
    • Utilize engagement strategies to build rapport with external community organizations and vendors.
    • Participate in a variety of outreach/marketing activities and events in the local community, including health fairs, foodbanks, senior buildings, churches, legal aid, and any place where prospective patients may benefit from learning about CFHC services.
    • Work closely with internal departments to support patient engagement and confirm the patient has been connected with necessary resources, SDOH assessments, referrals to internal services (care management/care coordination, WIC, IBCCP, Healthy Start), and referrals to external services.
    • Identify gaps in use of services, such as physician access, through discussion with patients.

    Requirements

    Education and Experience
    • a minimum of an Associates degree, or a minimum of three (3) of relevant experience required.
    • Certified Application Counselor license required within three (3) months of hire.
    • Senior Health Insurance Plan counselor certification required within three (3) months of hire.
    • Bilingual ability (Spanish) may or may not be required based on individual department needs.
    Knowledge/Skills/Abilities
    • Demonstrated ability to collaborate effectively in a team setting.
    • Ability to maintain effective and professional relationships with patients and other members of the care team.
    • Knowledge in public and private health coverage programs and Illinois Insurance Exchange.
    • Experience with Medicare, Medicaid, third-party insurance, copayments, etc.
    • Experience in working with a diverse population base preferred.
    • Ability to appropriately handle confrontation and conflict.
    • Ability to work independently and to seek consultation as necessary.
    • Ability to follow organization and departmental processes and procedures.
    • Ability to work in a high stress/fast paced/high work volume environment.
