    POSITION: Case Manager

    SALARY: $26.71-$30.96 DOE

    REPORTS TO: Clinical Supervisor

    LOCATION: BHP; Nespelem, Washington

    Basic Functions: The Professional Case Manager works autonomously, using discretion and independent judgement to provide clients, families, or vulnerable populations with medical and psychosocial support needed to cope with chronic, acute, or terminal illnesses. This position will also carry a caseload for maintenance for throughout the life of the case.

    Qualifications

    MINIMUM QUALIFICATIONS:

    Education and Training:
    • This position typically requires a Bachelor's Degree in Social Work (BSW) or Licensed clinical social worker (LICSW, LICSWA, LSWAIC), plus Certified Care Manager (required within 1 year of employment)
    • Requires a valid driver's license and be eligible for the Colville Tribes vehicle insurance program.
    • Must have a healthy life style for the past three years and maintain alcohol and drug abuse free throughout employment.
    Knowledge, Skills, and Abilities:
    • Requires specialized knowledge of the principles, practices and procedures associated with care management/ case management services.
    • Requires advanced knowledge of verbal and written communication with various types of ages.
    • Requires being able to work effectively with the public and handle conflicts, complaints, or problems professionally.
    • Requires knowledge of and ability to be sensitive to Native American culture and lifestyle.
    • Requires previous work experience with STRICT confidential information.
    • Requires the ability to establish and maintain effective working relations with the public, co-workers and various Tribal, State, and Federal Programs.
    • Requires ability to work under stressful situations and handle conflicts, complaints, and other problems in a professional and courteous manner.
    • Requires ability to adhere to professional and ethical codes of conduct and agree to standards of healthy lifestyle that includes abstaining from alcohol and non-prescribed chemicals throughout duration of employment.
    • Requires the ability to gather and analyze data and develop conclusions and recommendations.
    • Requires ability to plan, organize, and prioritize work processes in a high volume environment in order to meet schedules and timelines.
    • Requires the ability to communicate technical information and to interact with a wide variety of groups and individuals inside and outside the organization.
    • Must be able to communicate in a professional manner, in both formal and informal settings.
    • Requires the ability to work cooperatively with personnel service providers, families, and agencies.
    • Must be able to organize and prioritize work and manage time effectively.
    • Requires ability to maintain strict confidentiality of all program related matters ensuring client and personnel rights have not been compromised.
    • Requires the ability to work independently.
    • Requires the ability to exercise good judgement.
    • Requires sufficient ambulatory ability to walk to project locations, to stand for extended periods of time, and to bend and stoop for limited periods of time.
    • Requires sufficient arm-hand-eye coordination to operate a computer keyboard and pointing devises at a proficient level. Requires sufficient visual acuity to read technical documents and instructions.
    • Requires the ability to lift, push, and pull objects of medium weight (less than 50 lbs.) on an occasional basis.
    • Requires the ability to grasp, reach, push, and pull and move light weight work materials and supplies.
    • Requires sufficient arm, hand, finger dexterity to use a personal computer keyboard, multimedia, and other office equipment.
    • Requires sufficient hearing and speaking skills to project voice and communicate with staff, clients and other practitioners in small group settings and in one-on-one conversations.
    • May require working a non-standard schedule; before and after normal work hours.
    Note: Pursuant to Tribal Policy, if this position is safety sensitive it is subject to pre-employment drug testing. In addition, this position is subject to reasonable suspicion and post-accident drug testing.

    TRIBAL MEMBER AND INDIAN PREFERENCE WILL APPLY; PREFERENCE WILL ALSO BE GIVEN TO HONORABLY DISCHARGED VETERANS WHO ARE MINIMALLY QUALIFIED.

    If required of this position, you must possess and maintain a valid Washington State driver's license and be eligible for the Tribes' Vehicle Insurance. In addition, this position may be subject to pre-employment background clearances. If applicable, these clearances must be maintained throughout employment.
