We are seeking a motivated and compassionate Clinical Staff Pharmacist to join our Ambulatory Hematology/Oncology Pharmacy team,
serving our infusion centers in Nashua and Manchester,
New Hampshire.
Responsibilities
- Gathers a complete medication history
and reconciles medication lists as applicable. , - Completes comprehensive
a medication review recommendation ensure medications are necessary safe cost effective. , - Suspected adverse drug reactions records adherence gaps patient adherence.
