    Job Description:

    SUMMARY OF POSITION

    Provides direct completion of engagement and administrative duties for the Illinois Health Practice Alliance (IHPA) and Illinois Admission Discharge and Transfer (ADT) Platform Collective Medical. The Discharge Coordinator must understand Healthcare Effectiveness Data and Information Set (HEDIS) requirements as it relates to various stages of care for clients experiencing behavioral health hospitalizations, transitions in care, and ER visits. Responsible for completing engagement activities for clients and coordinating needed follow up activities within the organization or externally. Responsible for completion of all administrative tasks associated with IHPA. Maintains up-to-date knowledge of the transition requirements for individuals who have experienced a recent hospitalization, ER visit, or transition in care. Assists clients with personal recovery goals, helps clients develop natural support systems, refers/links clients with resources, and coaches clients to advocate for themselves within their healthcare systems.

    ESSENTIAL DUTIES & RESPONSIBILITIES
    • Establish relationships and practice core coaching skills to transform client values/desires into action and maintain lasting change over time. Educate clients on the purpose of IHPA required engagement activities. Provides taxonomy appropriate clinical services to clients via telehealth to close IHPA tasks. Complete safety plans and transition of care contacts with IHPA clients. Coordinate internal follow up for clients attributed to Collective Medical (ADT) Portal.
    • Provide solution focused brief interventions to IHPA members when needed.
    • Organizes administrative duties as assigned by the DQI (Director of Quality Improvement)
    • Completes all client engagement for IHPA assigned clients as tasks are triggered.
    • Coordinates with cross functional teams to ensure engagement and service activities are completed for clients within designated time frames.
    • Participates in IHPA organizational level transformation and development by participating trainings, committees, and pilots as requested by IHPA.
    • Prepares and participates in huddle meetings with IHPA leadership.
    • Coordinates with IHPA liaison to reassign members to correct IHPA facility. Coordinates with IHPA providers at other facilities to ensure quality of care.
    • Daily monitoring of IHPA and Collective Medical portals to coordinate timely follow up.
    • Work in consultation/collaboration with clients, family members and other providers to link clients with necessary resources or facilitate empowerment to access resources independently.
    • Empower clients to self-manage chronic health conditions. Assist clients with finding solutions utilizing motivational interviewing. Assist clients in overcoming any barriers to goals, including access to care. Assist with navigating health care systems. Link clients to community-based social support/services and develop collaborative relationships with other departments, services, and community health care agencies. Help clients learn/navigate virtual platforms to assist in the completion of video telehealth visits. Assist service recipient in making discharge plans and provide follow up services as assigned.

    KNOWLEDGE, SKILLS & ABILITIES
    • Effectively communicate via written, verbal, in person and virtual methods.
    • Demonstrates knowledge of IHPA requirements.
    • Knowledge of role and function of various community resources
    • Knowledge of internal and external referral procedures
    • Skill in customer service/experience techniques

    QUALIFICATIONS

    Education Level

    Minimum requirement is a mental health professional. Bachelor's Degree in human services related field or an individual with a high school diploma or GED with a minimum of five years documented clinical experience in mental health or human services. Other requirements for Mental Health professional status will be examined upon application submission.

    Years of Experience
    • Experience participating in their own personal recovery process in order to care for self or family member is preferred but not required.
    • Requires utilizing electronic health records and/or other automated/electronic documentation systems and to have computer and keyboarding experience and skills or the ability to develop such skills within required timeframes in order to meet the level of proficiency set by the agency.
    • Direct Service Providers utilizing electronic/automated documentation systems will be required to undergo a computer and keyboarding assessment test to determine proficiency.

    Certification/Licensure

    Must successfully complete all required background screenings.

    $15.86--$19.78
