- Supervisor: Medicaid Unwind Project Director
- Participate in training for the Medicaid Unwind project.
- Travel across Region 7 for outreach activities.
- Contact and provide information to impacted Medicaid beneficiaries.
- Setup outreach tables at FQHCs weekly.
- Engage with patients on Medicaid enrollment education.
- Process Medicaid applications and renewals at events and through canvassing.
- Report activities promptly to LDH.
- Identify challenges, resources, and best practices for project success.
- Research outreach event opportunities.
- Collaborate with various organizations to achieve project goals.
- Submit reports and attend meetings regularly.
- High school diploma required, baccalaureate degree preferred.
- Minimum 3 years of community outreach experience preferred.
- Community Health Worker certification.
- Computer literacy with Microsoft Office proficiency.
