Avera is seeking a Registered Nurse (RN) - Case Manager to join our Team

Job Summary

The RN Case Manager is a specialized nurse who identifies individuals who would benefit from case management for complex/chronic conditions and/or service coordination.
The RN Case Manager formulates and implements the member's care plan. Collaborating with physicians, population health personnel, employers, community groups, hospital-based case managers/utilization review personnel, clinic-based care coordinators/preauthorization personnel, and Insurance Division personnel, the RN Case Manager strives for the best possible outcome in a cost-effective manner following the standards outlined by the National Committee for Quality Assurance (NCQA).
The RN Case Manager should possess comprehensive knowledge of various aspects of all health plan business lines, covering commercial, marketplace, and self-funded clientele.
This role entails interaction with stakeholders in the health system, influential figures, vendors, delegated entities, healthcare providers, policymakers, and accreditation and government bodies.

Education and/or Experience

Graduate from an accredited RN program (Associate's degree or Bachelor of Science degree).
Four years of professional nursing experience, preferably in an acute care setting.
Prior experience in case management, disease management, care coordination, or equivalent is advantageous.
Proficiency in Microsoft Word, Excel, Access, and Power Point. Familiarity with data extraction tools is beneficial.

Licensure/Certification/Registration

Licensed as a Registered Nurse in South Dakota or complying with the enhanced Nursing Compact Licensure (eNCL) requirements is essential.
Being a Registered Nurse in Minnesota or obtaining the certification within 120 days of hire is mandatory.
Completion of certification in Case Management, Managed Care Nursing, or a related specialty within 2 years of employment is required.

Work Schedule

Monday - Friday; 80 hours over 2 weeks
- 8:00 am - 5:00 pm

Pay

The hourly wage range for this role is as follows. Actual pay rate is dependent on experience level.

$29.50- $44.25 per hour
