    Description

    Job Title: Team Manager

    Overview:

    The Team Manager is the leader of the Patient Care Team, ensuring patients and families receive continuity and quality care from admission to discharge or transfer to bereavement.

    Salary Range:

    $118,000 - $124,000 per annum

    Responsibilities:

    • Directs all patient care services, including volunteer and bereavement, as integrated components of the interdisciplinary plan of care.
    • Collaborates with team physicians and attending physicians as needed, serving as a clinical resource for the interdisciplinary team.
    • Participates in alive discharges to ensure effective discharge planning.
    • Oversees team schedules and coordinates services for adequate coverage of all patients.

    Staff Supervision and Management:

    • Interviews, selects, trains, supervises, evaluates, and dismisses team staff in conjunction with the Patient Care Administrator.
    • Ensures staff competence and performance levels through field visits, documentation evaluation, and provision of team-based services on Vitas values, hospice principles, and practices.
    • Acts as a resource and mentor for staff on clinical issues, documentation, inter- and intra-team problem-solving, and customer service behavior.
    • Reviews and approves payroll, ensuring accurate time, mileage, and additional expenses documentation.

    Quality Improvement/Regulatory Compliance:

    • Monitors clinical records and team activities to ensure accuracy, completeness, and adherence to required standards in accordance with Vitas policy and regulatory bodies.
    • Participates in performance improvement activities to address service issues, challenges, and documentation concerns.
    • Complies with the QAPI program and assists senior management with surveys and requests from regulatory bodies and intermediaries.

    Customer Service/Sales/Marketing:

    • Resolves service issues promptly and satisfactorily with patients, families, and internal customers.
    • Analyzes customer service issues to identify causes and works with team members to improve performance.
    • Regularly visits LTC and Contract Bed facilities to ensure care plan integration, customer satisfaction, and discuss criteria for determining appropriateness.

    Cost Containment:

    • Verifies treatments, staffing, and supplies.
    • Monitors resources and utilization of clinical operations, including labor, pharmacy, medical supplies, equipment, and mileage.
    • Approves all bills and invoices related to patient care services.

    Professional Development:

    • Attends in-services, educational seminars, and workshops.
    • Develops and achieves professional growth goals and objectives.
    • Participates as a mentor for newly hired Team Managers.

    Qualifications:

    • Reliable transportation with valid license and insurance coverage for driver and passengers.
    • Knowledge of primary medical and nursing care principles and oncologic nursing and palliative care.
    • Basic understanding of interpersonal dynamics within healthcare teams and knowledge of the process of dying and bereavement.
    • Working knowledge of Vitas information system (Vx) and management reports.
    • Two years' successful supervisory experience or equivalent in a healthcare organization.

    Education:

    • Bachelor's degree in Nursing or related field from an accredited college or university or international equivalent preferred.
    • Current and valid R.N. License to practice in the state where the Vitas program is located.
