    We are hiring IMMEDIATELY for a Hospice Licensed Practical Nurse to join one of our community mental health providers

    Salary: $67K - $81K; 5% target bonus per year

    Sign-On and Relocation available

    Schedule: Mon - Fri

    Locations:

    Wilsonville, Canby, Tigard and Tualatin

    Summary:

    The Licensed Practical Nurse assists in planning, coordination and care of hospice patients in accordance with the patient's plan of care. The position provides direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the interdisciplinary team to help achieve positive care outcomes.

    Required

    • Graduate of an accredited LPN training program
    • Minimum one (1) year of experience as a licensed practical nurse in home health, hospice, skilled care facility, memory care unit, or the acute care setting (preferably medical/surgical)
    • Current unencumbered LPN license in the State of Oregon
    • Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle
    • CPR certification at hire or within 6 months in position

    Knowledge

    • Knowledge of Federal, State and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations
