Northwest Health - Porter is seeking a Nurse Practitioner to work in a diverse role in both select community long term care facilities as well as the primary care office.

The goal is to ensure timely assessment and intervention as needed.

Ideal candidate has strong clinical experience and desire to work with patients of all ages in health maintenance and disease prevention.

Details of the position include:
Combination of both nursing home and primary care practice

Round in nursing homes for well visits and in response to patient needs

Opportunity to develop primary care practice for new and existing patients

Strong communication and follow through needed with nursing staff, physicians and

families

Porter's inpatient and ambulatory programs are accredited by Joint Commission and continue to be recognized nationally in multiple areas including award winning centers of excellence by American Heart Association, Blue Cross and Blue Shield, and the Leapfrog Group on quality and clinical safety practices; HealthGrades, the Society of Chest Pain Centers, and most recently, became the first in Indiana to receive full Atrial Fibrillation with Electrophysiology Services Accreditation status from the Society of Cardiovascular Patient Care (SCPC), an Institute of the American College of Cardiology.

Northwest Health - Porter hosts a team of experienced caregivers in the family-centered Women and Children's Pavilion. Longest-running NICU in northwest Indiana is onsite. The NICU admits infants of any gestation and weight. C-sections are performed in the delivery unit for convenience and safety of patients. Skin-to-skin delivery is offered in the C-Section suite.
