    Summary



    Patient Aligned Care Team (PACT) Registered Nurse is responsible and accountable for individual nursing practices and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. The nurse brings primary care principles and theory with limited application of critical thinking skills.



    Duties


    Duties include but are not limited to:

    • Delivering fundamental knowledge-based care to assigned clients while developing technical skills.
    • Assessing, planning, implementing, and evaluating care based on age-specific components.
    • Assuming responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care.
    • Administering medications and procedures per established policies and guidelines.
    • Influencing care outcomes by collaborating with members of the interdisciplinary team.
    • Responsible and accountable for individual nursing practice and determining the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care.
    • Providing gender-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
    • Managing assigned panel(s), utilizing registries, and ensuring appropriate evaluation and access is provided to patient panels.
    • Providing health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
    This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
