We are seeking a dedicated and compassionate Home Health Field PT Weekends to support our growing geriatric home health population in Austin, TX and the surrounding area. We offer competitive pay and wonderful team support.
+Responsibilities
- Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.
- Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
