Job summary
The Direct Care Professional – Individual Supports (IS) implements individualized programs as specified in Individual Service Plans, provides for individuals' health and welfare, and ensures general cleanliness and safety in individuals' homes.Responsibilities
Instruct and assist individuals with personal hygiene, housekeeping, social skills,advocacy use of leisure time response to emergency situations. Facilitate community integration by encouraging individual preference
and providing opportunities for recreational activities. Encourage and assist individuals to pursue relationships develop rapport with local community resources such as banks physicians stores etc. Instruct assist teach individuals meal planning meal preparation grocery shopping while encouraging the
individuals participation to the fullest extent Monitor individuals medical health assist in making doctor appointments provide transportation as needed advocate for
individuals when necessary Address immediate problems in the home or community notify the direct supervisor or on-call supervisor Assist individuals with financial management to degree specified in Individual Service Plan manage petty cash documenting providing receipts all money spent instruct physically
assist individuals housekeeping activities daily weekly basis promote sense pride ownership home When applicable ensure home safe free clutter debris Instruct administer medication monitor medication use Individuals documenting accordance Department Public Health policies procedures Work assigned schedule may include weekends holidays Comply reassignments directions division's supervisor director Work modified schedule when day programs closed site specific Participate staff meetings required training sessions including service training scheduled Facilitate house meetings Individuals May be required use employee's personal vehicle work purposes Perform duties assigned Maintain program Agency trainings
