    Description


    The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904.

    As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services.

    Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.


    Jordan Health is now seeking a
    Staff Pharmacist who, under the guidance of the Director of Pharmacy, is primarily responsible for the daily functions and operation of the pharmacy including dispensing, review, and verification of prescribed medications.

    The pharmacist will oversee and direct pharmacy technicians, interns, residents, and provide counseling and education to patients to enhance the customer experience.

    Requirements

    The
    Staff Pharmacist will ensure that:


    • Data entry, insurance billing and reconciliation of claims.
    • Performs prospective drug utilization review (DUR) for drug-interactions and incompatibilities.
    • Dispenses medications, performs final verification checks, and counsels patients on how to properly take their prescribed medications.
    • Provides all state approved immunizations and vaccines under patient and/or non-patient specific standing statewide and/or Jordan Health protocols.
    • Supervision of pharmacy technicians, students, interns, and residents when required.

    Education And Experience Required:

    • Bachelor of Science Degree in Pharmacy from an ACPE accredited school required.
    • Doctor of Pharmacy (PharmD) degree preferred.
    • Minimum of 5 years' experience as a staff pharmacist preferred.
    • 340B experience preferred.
    • Experience working in a federally qualified health center (FQHC) or Community Health Center preferred.
    • Bilingual (Spanish) desired.
    • Clinical pharmacy experience desired.
    • Experience with disease state management, precepting pharmacy students and collaborative work with primary care providers and nursing staff desired.

    Licenses and Certifications:

    • Licensed and currently registered through the NYSED as a pharmacist.
    • Immunization Certificate through the American Pharmacists Association (APhA)
    • Current BLS/CPR Certificate through the American Heart Association
    • Infection Control Certificate.

    Special Skills and Knowledge Required:

    • Strong customer service skills.
    • Excellent oral and written communication skills.
    • Outstanding interpersonal and organizational skills.
    • Acute attention to detail.
    • Ability to work independently and as a team in a fast-paced environment with frequent interruptions, occasional public contact, and occasional crisis situations.
    • Experience using appropriate technology and Microsoft Office tools for work-based communication.
    • Ability to learn the electronic health records and pharmacy software system.
    • Adaptability and flexibility in the workplace and being open to change.
    • Ability to maintain strict confidentiality with medical information and conform to HIPPA regulations.
