At our Health Center, a 96-bed skilled nursing facility, we provide in-patient residential care, short-term rehabilitation, and specialized services. With a low patient-to-nurse ratio of only 24 patients per nurse supported by 3 aides, we prioritize our residents' well-being through high-quality nursing care.

We are in search of full-time team members for our 2nd (3:00 pm - 11:30 pm) and 3rd shifts (11:00 pm - 7:30 am). PRN applicants are also encouraged to apply. Join us in our mission to offer exceptional care and support to those in need. Begin your journey with us today.

Job Duties:

Support leadership in ensuring 24-hour accountability for resident care
Provide direct patient care following physician's orders and established nursing procedures
Administer medications, treatments, and specialized therapies as prescribed
Conduct admissions, assessments, and documentation for all residents
Participate in care planning and maintain accuracy of care plans
Observe and enforce infection control measures
Maintain strict patient confidentiality
Document resident assessments and assist physicians as needed
Respond to emergency calls campus-wide
Perform assigned duties diligently

Requirements:

Graduate from an accredited nursing school
Valid license to practice as a Registered Nurse or Licensed Practical Nurse in Illinois
Current CPR certification
Knowledge of healthcare delivery in Geriatric, Rehabilitation, or Skilled Nursing settings
