- Master's Degree or equivalent experience with NP licensure
- Current RN and NP licensure with prescription authority
- Board certified in a Matrix approved specialty
- BLS, ACLS or CPR certification
- Minimum 1 year as Nurse Practitioner
- Strong computer skills and verbal/written communication abilities
- Comfortable with frequent change
- Ability to travel, valid driver license, and vehicle with insurance
- Travel to meet patients in their homes

About the Position:
During up to an hour-long visit, Matrix providers assess a member's health, medical history, social environment, and other factors. This in-depth assessment helps improve care quality and potentially address multiple health gaps in one visit.

Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $90.00 or $95.00 (based on county) per completed Assessment
Location: Home, SNF, Telehealth and community environments in Toledo, Ohio
Hours: Days, Evening, Nights, weekend, Flexible Scheduling
Benefits: PRN/Part Time providers receive 401(k) and limited Voluntary Benefits, Mileage Reimbursement plan
