    Baylor Registered Nurse / RN

    Revival Home Care Agency is seeking a passionate, dedicated Home Health RN to join our team in Annandale, VA.

    About the Role

    Your Contributions

    • You will make a difference in our patients' lives by providing safe patient care, delivering individualized nursing care plans to ensure our patients have an optimal care experience, and working as part of an interdisciplinary team to support both patients and their families.

    Location and Schedule

    Our location is Annandale, VA, with a coverage area of Loudoun County. The schedule is 30-40 hours per week, including weekends (3 10hr days, 2 12hr days, or 4 10hr days).

    Responsibilities

    • Attend and actively participate in weekly team meetings
    • Provide rehabilitation and coordinate medical needs for clients
    • Monitor and administer medications, order new medications, and discontinue medications as ordered by the physician
    • Assist with keeping within assigned nursing budget
    • Conduct assessments and approve plan of care and service delivery methods
    • Perform competency and skills evaluations on employees who provide direct care
    • Inform the physician and staff of changes in the client's medical or mental condition and needs
    • Write progress notes, summaries, and discharges
    • Implement treatment/rehabilitative activities and strategies
    • Report significant/major incidents or accidents and notify proper authorities of circumstances or situations that present possible health hazards
    • Follow consumer health plans and provide and promote consumer choice and facilitate independence
    • Assist in maintaining and updating nursing, pharmacy, infection control, and health and safety policies and procedures
    • Other tasks as assigned

    Qualifications

    • Graduate of an accredited school of Nursing
    • Current Registered Nurse licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
    • Bachelor's degree preferred, with one (1) year of home health experience preferred
    • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist
