Are you passionate about patient care? TeamHealth is looking for a nurse practitioner (NP) to work Monday through Friday doing primary care rounding in a skilled nursing facility in the Seaford-Millsboro, Delaware, area.

Perform primary care in a post-acute setting for short-term and long-term patients
Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
Consult with physicians and coordinate patient care with other healthcare practitioners
Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
Ability to collaborate with the healthcare team
Empathy, compassion and ability to connect and engage with your senior patients or residents
1+ years primary care experience required
~ Post-acute care experience a plus, but not required
