- Introduces families to the treatment process and to a strength-based and trauma informed approach to providing care and services.

- Documents the provision of Family Partner services including both face-to-face and non-face-to-face activities.
- Together with the families, help them to recognize their needs and the importance of allowing the team to develop individualized, strength-based treatment plans that reflect the family's goals and objectives.
- Assists in the identification of natural/non-traditional and community support systems, and links families to community resources.
- Collaborates with clients to develop an individualized care plan that is grounded in the family member's perspectives and includes items that reflect the family's values and preferences.
- Works in tandem with team partner the Intensive Case Manager to achieve team goals.
- Assists the family in selecting individuals to be on the family's team; these may include both formal and informal support.
- Participates in interdisciplinary team staffing. Presents psychosocial assessment finds. Updates team on progress or lack of progress in reaching agreed upon goals. Provides relevant information that might affect treatment.
- Follows-up on assigned consumers who miss scheduled appointments: Documents attempt to contact consumers: Reports repeated unsuccessful efforts to contact the treatment team.
- Follow-up within 24 hours on assigned consumers who access emergency services: Reports status-disposition to treatment team. 100% compliance expected.
- Attends all recommended Family Partner and Wraparound training events.
- Performs other duties as assigned.
- Conducts job responsibilities in accordance with the ethical standards of conduct, state contract, appropriate professional standards, and applicable state/federal laws.

- Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).

- Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain a Texas Driver License within three (3) months of employment.

The Family Partner advocates and promotes Metrocare Services from a primary caregiver's point of view. This position serves as part of the interdisciplinary Wraparound team in identifying and addressing the needs of the primary caregivers as it relates to the consumer. A requirement for the Family Partner position is for the individual to be a current or previous caregiver of a child who has a serious emotional disturbance. Family Partners should have a strong connection to the community and be very knowledgeable about resources, services and supports for families. The personal experience of the Family Partner is critical in earning the respect of the family and establishing a trusting relationship that is valued by the family. The position is 80% field-based and 20% clinic-based.
