- Perform clinical assessment and identify patient needs.
- Document, prepare, and complete patient Plan of Care. Track progress and revise as needed.
- Meet mandatory continuing education requirements.
- Apply concepts of infection control and universal precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
- Provide appropriate plan, treatment, and evaluation for speech, language, cognitive, and swallowing disorders with appropriate tests and measurements. Maintain pertinent records of specific tests performed and progress made.
- Communicate and educate family concerning patient deficits, ways to facilitate treatment, and appropriate discharge plans.
- Promote company mission and administrative policies to ensure the quality of care.
- Current Texas State License as Speech Language Pathologist
- Current Texas Driver's License and reliable transportation
- Six months of experience as Speech Language Pathologist in an acute care or rehabilitation setting. Pediatric experience is preferred.
- Effective interpersonal relations and communication skills
- Knowledge of durable medical equipment
- Ability to work a flexible schedule and travel locally
- Able to work in a patient's home in various conditions
- Excellent organizational and time management skills
- Proficient in Microsoft Office
- Able to maintain a high level of confidentiality
- Proof of current CPR certification
- Ability to do heavy lifting, bending, pulling, pushing, and standing. Prolonged standing and walking are required.
