As a Physical Therapist (PT, DPT, RPT) specializing in Home Health, your responsibilities will include assessing patient care needs related to various aspects of their health, such as mobility, endurance, and pain management. You will work closely with patients to create personalized treatment plans, implement interventions to enhance their well-being, and monitor their progress.

Assess patients' functional status and therapy requirements, collaborating with physicians to develop care plans
Observe and communicate patients' responses to treatment to the medical team
Provide therapy to alleviate pain, improve function, and help patients maintain their highest level of independence
Educate and update healthcare team members, patients, and families on care plans and progress

Licensure Requirements:

Active Physical Therapy license in the state of practice
Current CPR certification
Valid driver's license, vehicle insurance, and access to a reliable vehicle
