Alameda Health System is seeking a Director of Inpatient System Care Management to oversee the operations of inpatient Care Management across all AHS facilities. This role involves ensuring smooth patient flow from admission to safe discharge.
Essential Job Functions:
Minimum Qualifications:
Master's degree in Nursing, Social Work, or related field
7-10 years of case management or quality experience, including managerial roles in healthcare organizations
N/A
Required Licenses/Certifications:
Active licensure as a Registered Nurse in the State of California or valid Clinical Social Worker license
Active BLS Certification
CPI Certification
Other certifications as required by department
Preferred Licenses/Certifications:
Case Management Certification (CCMC or ACM)
