    Director of Inpatient System Care Management at Alameda Health System

    Role Overview:


    Alameda Health System is seeking a Director of Inpatient System Care Management to oversee the operations of inpatient Care Management across all AHS facilities. This role involves ensuring smooth patient flow from admission to safe discharge.



    Essential Job Functions:


    • Accountable for Patient Care Coordination activities including Case Management, Care Coordination, and Discharge Planning
    • Aligns Case Management with AHS strategic goals
    • Optimizes Care Management processes and roles, driving process improvements
    • Monitors and evaluates inpatient Care Management Metrics; manages data collection and analysis related to healthcare resource utilization
    • Develops and implements Case Management policies in compliance with regulatory agencies
    • Collaborates with VP of Care Management and Physician Advisors on audits and reviews
    • Oversees educational needs and plans for the Care Management division
    • Recruits, trains, and coaches management staff
    • Supports financial analyses, denials management, and effective resource utilization
    • Coordinates the AHS Utilization Review Committee
    • Oversees patient, physician, and staff satisfaction
    • Provides education to physicians and staff on various healthcare guidelines
    • Serves as a liaison to community agencies and primary care providers
    • Coordinates and implements action plans for improving patient flow and care coordination
    • Drives improvements in department procedures and equipment
    • Performs assigned duties effectively

    Minimum Qualifications:


    • Required Education:

    Master's degree in Nursing, Social Work, or related field


    • Required Experience:

    7-10 years of case management or quality experience, including managerial roles in healthcare organizations


    • Preferred Experience:

    N/A


    Required Licenses/Certifications:


    Active licensure as a Registered Nurse in the State of California or valid Clinical Social Worker license
    Active BLS Certification
    CPI Certification
    Other certifications as required by department


    Preferred Licenses/Certifications:


    Case Management Certification (CCMC or ACM)
