Perimeter Behavioral of Springfield is daily committed to improving patients' lives by using a patient centered collaborative approach to provide high quality care addressing both the mental health and physical wellbeing of our patients.
Provides coordination and acts as liaison with community based social and mental health agenciesFacilitates discharge planning and conducts post discharge follow-up to assure that linkage of the patient with community resources has occurred reducing re-hospitalization

LMSW or LPC (preferred)
Experience:
At least one (1) year of related experience in a hospital setting with psychiatric experience preferred
Successful completion of CPR and Crisis Prevention Intervention Training within 30 days of employment
Schedule:
Seeking FT Monday-Friday with some weekend availability
