Description:
The Rehab Patient Navigator is responsible for the navigation advocacy of patients during the inpatient rehab admission.
The Navigator ensures patients are oriented to the rehab unit including rehab requirements and expectations by day 2 of admission, acts as a liaison between the patient/caregivers and the interdisciplinary team to communicate pertinent needs and information, assists with plans for discharge to help reduce skilled nursing facility (SNF) admissions, decrease length of stay, and facilitate a smooth and successful discharge to the community.
The Navigator will act as a resource for patients/caregivers to contact when they have questions or concerns and will coordinate transitions between the phases of care.
Required Education, Experience and Certification/Licensure
High School diploma or equivalent
3-5 years of direct patient care on inpatient rehab unit and/or other related experience.
Required Skills and Abilities
Excellent communication and organizational skills.
Demonstrates excellent interpersonal skills with patients and medical staff.
Able to function autonomously, be self-directed, and utilize good judgement and decision-making.
Demonstrates proficiency in computer data bases and software to effectively perform role.
Demonstrates ability to effectively problem solve with internal and external customers.
