    Meaningful Work Opportunity at Home

    Become a Host Home / Shared Living provider with Mosaic at Home and enjoy an average annual income of $80,000 (federally tax-free) plus room & board.

    To qualify, you must reside in the state of Iowa due to state funding and own or rent your home. Please note that we are no longer taking new inquiries in the Des Moines and surrounding Metro areas.

    Responsibilities of a Host Home Provider

    • Provide support for daily tasks such as cooking, cleaning, laundry, budgeting, decision-making, etc.
    • Cultivate life skills and foster independence in the individual you support.
    • Assist with activities of daily living, both at home and in the community.
    • Participate in medical appointments and provide support for overall health and well-being.
    • Document daily progress via web-based case management application.
    • Plan fun and leisure experiences to promote community inclusion, integration, and socialization.

    Possible Additional Responsibilities

    • Assistance with personal care needs such as bathing, toileting, feeding, and behavioral support.
    • Support for medical protocols, including seizure, fall, and feeding tube management.
    • Care for individuals with complex physical or medical needs, such as hearing or vision impairment.

    Requirements for Becoming a Host Home Provider

    • Residency in the state where you wish to provide contracted services.
    • Valid driver's license, car insurance, and current vehicle registration.
    • Active home/renters insurance and ownership or rental of your own home/apartment/condo/townhouse.
    • Must be at least 18 years of age and have reliable computer and internet access.
    • Completion of all state-mandated trainings and ability to pass State/Federal and APS/CPS background checks.
