Job Summary:
The Intake Coordinator responds to inquiry calls, conducts initial intake assessments, provide referrals and coordinate admissions process according to Joint Commission regulations.
- Responds to inquiry calls and documents follow up calls made regarding pending assessments.
- Conducts assessments at facility or other locations and completes Comprehensive Interdisciplinary Assessments Initial Triage thoroughly.
- Refers patients who are not appropriate for facility programs to appropriate resources.
- Bachelor's Degree in Psychology, Social Work or related field required
- Must possess interpersonal skills to develop rapport with presenting patient family members and referral sources
- Must demonstrate knowledge of mental illness chemical dependency family dynamics which accompany them
