- Two (2) years home health nursing experience, minimum.
- At least two (2) years healthcare experience in the field of supervision including one year supervisory experience in home care or a closely related service.
- BSN in Nursing Preferred. Associate in Nursing minimum required.
- Acceptance of philosophy and goals of Agency.
- Ability to exercise initiative and independent judgment.
- Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal.
- Must be proficient in Axxess EMR Software

Seeking an Experienced Home Health Nurse to supervise Home Health Services in accordance with the Physician Plan of Care. The Nurse Manager participates in activities relevant to the direct provision of home health services. The Nurse Manager is responsible for compliance with CHAP and IPI policies and procedures, Federal and State Regulations, IDPA, IDPH, Medicare, payer and accrediting rules, regulations, and standards.
Job Description:
The clinical manager is responsible for delivery of nursing services in accordance with the physicians plan of care. This position provides leadership and supervision to nursing staff and home health aides to assure that the quality and efficiency of services delivered is in accordance with the agencys philosophy, objectives, policies and procedures and applicable regulations. The manager serves as the supervising nurse, in accordance with Illinois home health agency licensure requirements.
