Job Summary and Qualifications

The RN Care Coordinator  works collaboratively across teams to enhance quality of care, improve outcomes, and optimize services for PACE participants. This role focuses on coordinating care for high-risk participants—those with complex medical, mental health, or psychosocial needs—to reduce hospitalizations and emergency department visits. Your responsibilities will include:

• Care Coordination & Compliance
• Data & Quality Management 
• Professional Standards
• Team Engagement & Other Duties 

What qualifications you will need:

Required Education: Bachelor of Science in Nursing OR enroll in a BSN or advanced degree program within 6 months AND obtain a BSN within 4 years of hire date into RN position. Previous signed BSN commitment letter OR grandfathering will be accepted between departments and Mission Health entities. Exemptions may be approved with CNO System Council approval. 

Required License/Certifications: 

• Must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse Licensure Compact) license may apply, licensee should confirm with NCBON. 
•  BLS Healthcare Provider 
• Valid NC Driver's License. 

Preferred License/Certifications: National certification in area or specialty

Required Experience: One year registered nurse experience 

Preferred Experience: Home health nursing or case management experience
