Creekside Transitional Care & Rehabilitation in Meridian, ID, is a skilled nursing facility with long term care, post-acute rehab, and geriatric outpatient therapy services provided.

Licensed Physical Therapist
This position is open to PTs at all experience levels. New Grads Welcome
