    JOB DESCRIPTION SUMMARY
    :


    Under limited supervision, the Manager of Mobile Hospice Response Team (MHRT) plans, implements and manages all administrative, personnel, and operational activities of the Mobile Hospice Response Program.

    This includes supervision of MHRT nurse liaisons and Liaison nurses assigned to MHRT and partner hospitals., They work closely with the Medical Director in the development, implementation and administration of clinical operations, policies and procedures pertaining to the MHRT.

    The Manager of MHRT develops and coordinates educational activities for both internal and external staff.

    They will also serve as primary care provider for selected patients on behalf of Gilchrist who are enrolled in hospice in the MHRT partner hospitals.

    Education
    Successful completion of certified nurse practitioner program.

    Experience
    3year of experience functioning as a nurse practitioner in a clinical setting.
    Prior experience in a supervisory role is desirable.

    Skills
    Broad knowledge of medical treatment practices, especially internal medicine, geriatrics, and oncology,
    Knowledge of healthcare regulatory agency standards for hospice/ health care organizations.
    Knowledge of The Joint Commission (TJC) and CMS principles and standards.
    Familiarity with Part A Hospice Medicare and insurance coverage processes preferred.
    Ability to operate independently and to balance multiple priorities.

    Strong leadership, interpersonal, and influencing skills necessary to interact effectively with physicians, payers, regulatory agencies, staff, and other health professionals.

    Skill in using computers and personal productivity applications.
    Skill in written and oral communication.
    Ability to interact effectively with patients and families.
    Skill in reviewing, analyzing, and developing clinical summaries of patient electronic medical records.

    Licensures, Certifications

    Current licensure as an adult nurse practitioner by the Board of Medical Examiners of Maryland/Maryland State Board of Examiners of Nurses.

    Completion of health care CPR course annually.

    Physical Requirements
    Ability to walk and stand for extended periods of time.

    Ability to concentrate and pay close attention to detail for up to eighty percent of work time in performing duties and dealing with patients and families, often during stressful situations.

    Working Conditions
    Normal office work environment and patient care environment, including health care facilities.
    Exposure to adverse working conditions due to performance of certain patient care activities.
    Exposure to communicable disease.
    Position requires use of personal automobile and local travel in the Gilchrist Greater Living service areas.

    Conditions of Employment

    Must maintain productivity and quality standards as set by Gilchrist.
