Description
The Mass General Hospital Division of Palliative Care & Geriatric Medicine is seeking a competent and compassionate Clinical Nurse (RN) to join the Geriatric Medicine Service.
The Service provides primary care and consultative services utilizing the Five "M" Geriatric Care Model (multi-complexity, medication, mentation, mobility, and what matters most).
We serve a diverse, medically complex, and socially vulnerable population.
The Clinical Nurse is a licensed registered nurse who is responsible for ensuring competent, compassionate, individualized, nursing care for a specific population of patients and caregivers, including delegation to and supervision of non-licensed clinical support staff.
The Clinical Nurse collaborates with the Nursing Director, Medical Director, NP Team Lead, and other members of the patient care team to achieve optimal patient outcomes and practices in accordance with established institutional and departmental policies & procedures and standards of practice.
The Clinical Nurse also collaborates with the patient toward meeting the needs and expectations of the patient and family.The Clinical Nurse ensures that care is safe, efficient, effective, timely and meets the cultural, spiritual, and ethnic needs of each patient and family.

Qualifications
LICENSES, CERTIFICATIONS, and/or REGISTRATIONS (if applicable):
Current licensure as a Registered Nurse in the Commonwealth of Massachusetts.
Specialty certification preferred.
EDUCATION:
Graduate of an accredited school of nursing program.
BSN preferred.
EXPERIENCE:
Relevant clinical experience with the specialty (geriatric) with a strong preference for prior ambulatory clinic and/or visiting nurse/home care experience.
Two years of clinical nursing experience required.
