Make a DifferenceThe Director of Case Management is located at Community Hospital of Anderson.The Director provides onsite leadership to ensure that daily department operations function effectively and efficiently to meet the needs of all customer and regulatory groups.
This position will provide consultation/direction to staff to facilitate compliance with departmental processes & policies as well as various regulatory agencies.
This position provides direction, leadership, and guidance to employees and customers and facilitates continuous change and improvement. The Director will evaluate and monitor operational issues and make recommendations to promote cost-effective, quality care.Exceptional Skills and QualificationsApplicants for this position should demonstrate the ability to effect change by assuming leadership roles in interdisciplinary teams and supporting the growth and development of team members.
Graduate of an accredited School of Nursing and/or Social Work.If a Registered Nurse, a Bachelor's Degree is required with a Master's Degree required within 5 years of assuming the role.
If not a Registered Nurse, a Master's of Social Work (MSW) is required.Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) or current LSW or LCSW from the State of Indiana for Social Work candidates.
Three (3)Understand the relationship of individual patients, units, and/or departments to the network, to facilitate achievement of optimal goals within those relationships.
