- Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, referrals, case management, follow-up, and documentation
- Collaborate with healthcare professionals and caregivers
- Work in various settings including Home Visits, Facility Visits, and Telehealth Visits
- Master's Degree or equivalent experience with NP/PA licensure
- Current RN and NP/PA licensure with prescription authority
- Board certification in a Matrix approved specialty
- BLS, ACLS, or CPR certification
- Minimum 1 year experience as a Nurse Practitioner/Physician Assistant
- Strong computer skills, EMR software familiarity
- Excellent communication skills
- Positive, team player with flexibility
- Valid driver's license and ability to travel
- Travel to meet patients

Matrix providers conduct comprehensive health assessments during visits, gaining valuable insights into members' health and well-being. This approach enhances the quality of care and helps address multiple gaps in care with a single visit.

Matrix Provider – What to Expect:
Nurse Practitioner or Physician Assistant - PRN/Part Time - Home Risk Assessments
Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $90.00 or $115.00 based on county per completed Assessment
Location: Home, SNF, Telehealth and community environments in Fredonia, New York
Hours: Flexible scheduling including days, evenings, nights, and weekends
Benefits: PRN/Part Time providers receive 401(k) and limited Voluntary Benefits, Mileage Reimbursement plan
