    Registers Nurse/ Licensed Practical Nurse - Home Health

    Reports To:
    Clinical Supervisor or Agency Manager


    REGISTERED NURSE JOB DESCRIPTION SUMMARY:
    The registered nurse plans, organizes, and directs home care services and is responsible for overseeing the clients/consumers/patients' services.

    PATIENT CARE Completes an initial home assessment of patients/clients.

    Conducts supervisory home visits as assigned, assessing clients condition, and progress towards goals, providing patient care, teaching, and other skilled interventions as ordered.

    Initiates the plan of care, makes necessary revisions as patients status and needs change, Regularly re-evaluates nursing goals as part of nursing visits.

    Evaluate the clients needs during the initial face-to-face visit and subsequent visit. Prepares home care clinical notes and updates supervisor as needed.

    Communicate with the physician regarding the patient's needs and report any changes in the patients condition:
    Obtain/receive physician orders as required. Teaches clients and family/caregiver self-care techniques as appropriate. Attends Home care case conferences and other/interdisciplinary group meetings as requested.


    Wound Care:
    Assessing and monitoring wounds
    Cleaning and bandaging wounds
    Educating staff nurses on wound care protocols
    Creating, updating, and managing care plans
    Performing specialized procedures (e.g., removing dead tissue/debridement, cleaning wounds, and bandaging)
    Working with a multidisciplinary team to plan skin care interventions
    Recording patient information such as vital signs, allergies, medications, and medical history.

    ADDITIONAL DUTIES Participates in on-call duties as defined by the on-call policy Instructs and evaluates HHA care provided per policy Assumes responsibilities per personal growth and development and maintains, and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes Actively participates in Quality Improvement team and activities Assist in monthly staff in-services

    POSITION QUALIFICATION Registered Nurse with a current license to practice professional nursing in the state of Georgia.

    Maintains a current CPR Minimum of two years experience, at least one (1) is in the area of home health care.

    Self-directed and able to work with minimal supervision Demonstrates excellent observation, problem-solving, verbal and written communication skills, and nursing skills per competency checklist Shows the ability to organize and prioritize workload independently Management experience is not required.

    Responsible for supervising HHA clinically. Prolonged or considerable walking or standing. Able to lift position or transfer patients. Considerable reaching, stooping, bending, kneeling, or crouching. Visual activity and hearing to perform required nursing skills.
