    Requirements:

    Current unrestricted AR RN license, BLS certification, strong assessment skills with critical thinking and high level of problem solving within a healthcare facility.

    Strong documentation and communication with the ability to stay calm and cope with human suffering/emergencies. Prefer hospice experience, home health, or acute care nursing experience with computer charting experience. Position requires use of personal vehicle, a valid driver's license and current auto insurance.


    General Description:
    The Admission Nurse is an integral part of the Interdisciplinary Team and works closely with the Medical Director. Primarily responsible for coordination and execution of the admission process into Hospice care. Nurse will need to be able to effectively communicate the Hospice concept to families and patient. Will develop the plan of care and effectively communicate the patient/family care needs to the team, must be able to train and educate patient/caregivers, and establish an appropriate frequency of disciplines to meet the patient/families' needs. Must be able to deal with human suffering while maintaining professional boundaries. Required to be flexible and change schedule at a moment's notice to meet patient needs. Required to travel daily and manage own schedule effectively. Must have excellent clinical skills and be able to identify problems, perform head to toe assessment, educate and consult with the physician, and execute orders to meet patient's needs.
