The Intensive Case Management ICM Team Lead plans manages and directs the daily operations of Highland Rivers Behavioral Health Intensive Case Management ICM Team which is a medium intensity community based program for adults with severe and persistent mental illnesses The Team Lead provides direct care in a recovery focused environment The Team Lead provides direct care services including behavioral health assessments treatment plan reviews and treatment planning Duties and Responsibilities include Plans goals and objectives in accordance with accreditation standards governing boards and individuals in daily operations of assigned areas Participates in clinic treatment team meetings weekly Participates in quarterly coalition meetings conducted by DBHDD Participates in developing managing maintaining a collaborative cooperative service environment in the community at large Monitors ICM Team services to ensure compliance with fidelity accreditation standards governing boards individuals daily operations assigned areas Provides oversight completion monthly reports internally Highland Rivers externally Regional Office DBHDD etc Provides clinical oversight all clinical program operations staff addition providing clinical support staff call event licensed clinician needed Responsible keeping charts compliance ASO QI CARF DBHDD HFR other accreditation standards Establishes professional ethical relationships individuals families co workers Recruit hire train new team members perform standards outlined performance evaluations Assists staff reaching identified productivity target due Pay Performance Model Displays flexibility cooperation positive attitude Completes assigned tasks timely manner requests directions supervision appropriate Uses initiative judgement assure circumstances requiring attention referred appropriate team member Performs duties directed supervisor Program 24/7 service rotating on call possible weekend holiday work Benefits Health Insurance State Health Benefit Plan GA Flexible Benefits dental vision life critical illness retirement plan employer matching weeks Paid Time Off increase accruals years service paid holidays personal day Qualifying employer Public Student Loan Forgiveness NHSC loan forgiveness qualifying counties Clinical supervision licensure track
