Case Manager - Registered Nurse

• 13 week contract
• 40 hours per week - 5x8's
• Shift - Day shift and Night shift available - every other weekend
• Must have 2 years of RN experience and minimum of 1 year in Case Management
• Must have active CA license and BLS

```Duties```

• Provide comprehensive case management services to patients in a healthcare setting
• Conduct assessments and develop individualized care plans for patients
• Coordinate and facilitate communication between healthcare providers, patients, and their families
• Monitor patient progress and evaluate the effectiveness of treatment plans
• Collaborate with interdisciplinary teams to ensure continuity of care
• Utilize utilization management techniques to optimize patient outcomes and resource utilization
• Provide patient education and support to promote selfmanagement and adherence to treatment plans
• Document all patient interactions, assessments, and interventions accurately and timely

```Requirements```

• Valid Registered Nurse (RN) license
• Strong knowledge of physiology and medical terminology
• Ability to provide compassionate and patientcentered care
• Excellent communication and interpersonal skills
• Strong organizational and time management skills
• Ability to work independently as well as part of a team
